Atrial Arrhythmias

Case 1 Answer

PAC

History:
This ECG was recorder from a 60 year-old lady with a 2/7 Hx of cough, SOB and pleuritic chest pain. She was told by her GP that she has mild hypertension, which requires no antihypertensive treatment. She takes 150mg aspirin, nil other Rx.

ECG shows:

  • NSR, rate 84/min
-RR and p-p intervals are in equal length (small arrows)
  • Conducted PAC (arrow), preceding T wave is distorted by the premature p wave, followed by an noncompensatory pause.
  • LAD, Normal PR interval, QRS duration, QT interval for rate.

*If the pause is compensatory, the sinus beat after the PAC should have a measurement of 2 RR intervals to the sinus beat before the PAC. If the measurement is less than 2 RR intervals, then the pause is noncompensatory.

Chest X ray confirmed a LLL consolidation, she was treated with IV Amoxycillin.
