History:
This ECG was recorded from a 90 year-old man presented with 1/52 of SOB and lower limbs swelling. He has a long standing history of asthma and 40 years history of smoking.

ECG shows:

  • Sinus Tachycardia, 105/min, Right axis deviation, Baseline wander
  • Dominant R wave in V1, deep S waves in lateral leads. Suggestive of RVH
  • Features of right atrial hypertrophy, tall P waves in inferior leads.
