Regular Atrial Ectopics

History:
This ECG was recorded from a 50 year-old lady post-gall bladder surgery.

ECG shows:

  • Sinus rhythm with atrial bigeminy
         -Each sinus beat is followed by an atrial ectopic beat.
         -Mean ventriuclar rate, 78/min
  • Key to diagnosis here is the p wave morphology in lead II, bigeminal beat has a different QRS morphology to the sinus beat.
  • Poor R wave progression and mild ST elevation in V1-4, ? past anterior MI
